Antiplatelet Agents, Anticoagulants: New Medical Strategies

چکیده
The pathogenesis of stroke Stroke is a heterogeneous condition in which 85% is caused by arterial acute occlusion or ischaemic stroke and 15% blood vessel rupture or cerebral haemorrhage. Of those with ischaemic stroke, further heterogeneity is evident with cerebral ischaemia caused by artery-to-artery emboli, in situ small vessel disease or cardiac emboli. The underlying pathogenesis can usually be determined after considering the clinical phenotype and the results of this series of predominantly image-based investigations. This may result in a classification such as TOAST, whereby about 30% of ischaemic stroke is usually caused by artery-to-artery embolism, 20% by cardiac embolism and about 15% plus more artery disease. Regardless of the underlying cause of vessel occlusion, a common final pathway involves other platelet aggregation and/or stimulation of the coagulation cascade. Hence, preventative strategies are usually focused on these two basic mechanisms.

Bleeding in patients using new anticoagulants or antiplatelet agents: risk factors and management.
The most important adverse effect of antithrombotic treatment is the occurrence of bleeding. In case of serious or even life-threatening bleeding in a patient who uses anticoagulant agents or when patient on anticoagulants needs to undergo an urgent invasive procedure, anticoagulant treatment can be reversed by various specific strategies. Heparin and heparin derivatives can be counteracted by ...

Cryptogenic stroke in patients with patent foramen ovale.
Paradoxical embolism through a patent foramen ovale (PFO) is a commonly identified potential mechanism of ischemic stroke, especially in young patients. The recurrent stroke rate among young cryptogenic stroke patients with a PFO is modest, but higher than normal for their age. Co-existing atrial septal aneurysm is a substantial, and the only definite, potentiator of stroke risk in patients wit...
متن کاملHypersensitivity reactions to modern antiplatelet and anticoagulant drugs
Anticoagulation and antiplatelet drugs are among the most commonly used medical drugs. In addition to the long-established heparins, hirudins, coumarins and antiplatelet drugs such as acetylsalicylic acid, numerous novel and predominantly synthetic pharmacologic agents have come onto the market in recent years. These new agents act at various sites in coagulation and have significantly broadene...
متن کاملSynthesis of N-arylmethyl Substituted Indole Derivatives as New Antiplatelet Aggregation Agents
A number of N-arylmethyl substituted indole derivatives have been synthesized and their effectiveness against ADP and arachidonic acid induced platelet aggregation in human plasma was determined. The desired compounds were synthesized by reacting the appropriate aniline derivative with isatin (or substituted isatin) to form the corresponding imine structures. The so formed compound was then act...
